This document shows how to use Admix in iKnowMed Generation 2 to combine additives to Pre and Post hydrations. This is especially helpful for practices with an Orders Interface with MAR Updates enabled, where Lynx is sending the Dispense Details to iKnowMed Generation 2, as this information is used when calculating the Charge Capture Report. The below must be completed prior to Queuing and Dispensing from Lynx.

Pre and Post Hydrations

This section shows how to combine additives to Pre and Post Hydrations. In this example we will add a Potassium and Magnesium to each Hydration.

1.     Open the patient chart in iKnowMed Generation 2 and click the Admix tab.

2.     Select the Date for the Unplanned order on the left, which shows the order on the right.

3.     Click the ACTION button and select Move. Choose Magnesium Sulfate the click as shown below to the Pre-Hydration section. Immediately, this will move the Sodium Chloride with the Magnesium. Click Move


4.     You should see the Magnesium is now combined with the Sodium Chloride.

5.     To move the Potassium with the Sodium Chloride and Magnesium, click the ACTION button for the Potassium Chloride and select Move. Choose the option in the Pre-Hydration section and click the MOVE button.

 

6.     To verify, all three items are listed together under Pre-Hydration, as shown below.

7.     Follow the same instructions for the Post Hydration.

8.     Pre and Post Hydrations should be as presented below. Please follow Practice Policy for adding:

a.  Amount in mL    b.  Admix Fluid    c.  Admix Fluid Volume    d.  Total Volume    e.  Administer Over

Note: Do not enter Waste, as this information will come from Lynx.

9.     Back in iKnowMed Generation 2, click MARK ALL AS PLANNED >> APPLY SPECIAL SCHEDULE.

10.   Choose the appropriate choice to apply to future cycle days. Click OK. Print Labels.


11.   Log in to Lynx, proceed to the Dispense page, and search for your Patient. Search by Name, ID (MRN), or DOB.

12.    You should see the orders via the Lynx Dispense tab. The first section will be for the Pre-Hydration.

 

13.   Scroll down past your Premedications and PRN medications for the Post Hydrations.

Note: There is no header in Lynx to show Pre vs Post Hydrations, so be very careful when dispensing to avoid issues in the MAR in iKnowMed Generation 2.

 

14.   You should see the below on your Dispense. If your practice doesn’t dispense fluids from Lynx, please exclude that from your process.

Note: You will use one vial of the Potassium Chloride to pull the two doses of 10 MEQ.

15.   The below is how your Lynx Queue should look prior to Dispensing from Lynx.

Note: Your workflow could be that you dispense the POST Hydration later.

16.   The final review is in Admix Planning in iKnowMed Generation 2. The Pre and Post Hydrations will have the appropriate information from Lynx (NDC, Dispense Amount, Waste, and Lot # and Expiration Date if you are tracking them in Lynx).
