OB/GYN History is arranged into several sections.

Menses Details

Enter the patient's age when they had their first period in Age of First Menses.

For Last Menstrual Period, choose when the patient had their last period.

Enter their typical Menstrual Cycle Length.

Pregnancy Details

Choose an option that best describes whether they are Currently Pregnant. To clear this selection, click Clear.

Choose an option that best describes their Desire for Fertility. To clear this selection, click Clear.

In Gravida, enter the number of times the patient has been pregnant.

For Para, enter the number of births they have had after a pregnancy reaches 20 weeks of gestation.

Enter the number of miscarriages (a.k.a. spontaneous abortions) they experienced in SAB.

Enter the number of therapeutic abortions they experienced in TAB.

Enter the patient's age when they had their first live birth in Age at First Live Birth.

In Number of Term Births, enter the number of early and full term births the patient has had.

For Number of Pre-term Births, enter how many pre-term births the patient has had.

Enter how many living children the patient has in Number of Living Children.

Choose an option that best describes their History of Breastfeeding. To clear this selection, click Clear.

Menopause Information

Choose your patient's current Menopause Status. To clear this selection, click Clear.

For a Postmenopausal patient:

Enter their age when they entered menopause in Age at Menopause.
For Reason, choose how they entered menopause.
For Symptoms, click each Symptom they experience. To remove a Symptom, click the Remove (X) icon next to the Symptom.

Ob/Gyn Medication History

Contraceptive Hormonal Use

If your patient has used a hormonal contraceptive:

Enter the Number of years of use.
If they are currently using a hormonal contraceptive, choose Yes for Currently using. Otherwise, choose No.
Enter the Date of most recent use.

If your patient has not used a hormonal contraceptive, choose No for Contraceptive Hormonal Use.

To clear Contraceptive Hormonal Use, click Clear.

Post-Menopausal Hormonal Therapy

If your post-menopausal patient has had Post-Menopausal Hormonal Therapy:

Enter the Number of years of use.
If they are currently using a hormonal contraceptive, choose Yes for Currently using. Otherwise, choose No.
Enter the Date of most recent use.

If your patient has not used a hormonal contraceptive, choose No for Post-Menopausal Hormonal Therapy.

To clear Post-Menopausal Hormonal Therapy, click Clear.

Other Contraceptive History

If your patient started using an intrauterine device (IUD), check Initiation of IUD.

If they used other contraceptive methods:

To clear Other Contraceptive History, click Clear.
