Click the Stage at Diagnosis. This will populate from the problem list.
Click on the drop-down arrow in the My Diagnosis box to select the treatment diagnosis.
Click in the Current Status of Disease and select from the drop-down options.
Click the My Prognosis box and select from the drop-down options or enter free text.
Click The Goal of My Therapy is box and select from the drop-down options or enter free text.
Edit or add information using the provided drop downs or manually entering data into the text fields.
